To request a police report, complete a "Request for Documentation" form. These forms are available at the Lafayette Police Department Records division between 7 AM and 5 PM Monday through Friday. Your request gets reviewed consistent with state and federal statutes. You should get a response within one week. Be as specific as you can with dates, names, and case numbers.
If you need report information for insurance purposes, the records division will provide a summary letter for a $5 fee by check or money order payable to the City of Lafayette. Record checks cost $7 and require valid identification. You must complete the records check form at the Lafayette Police Records Section at 601 Columbia Street between 8 AM and 4:30 PM. Record checks take 24 hours to process and only cover arrests made by the Lafayette Police Department.
